Please call to discuss current fees. All fees are due at the time of service. Please provide at least 48 hours notice for any cancellations or changes. If you do not attend your scheduled appointment and you have not provided notification at least 48 hours in advance, you will be required to pay the full cost of the session.
Dr. Hipolito is a participating provider with Anthem Blue Cross and Cigna. If you have other coverage and would like to use out-of-network (PPO) benefits, the necessary paperwork will be provided to you to file a claim for reimbursement.
